Our Client is seeking motivated and reliable Case Managers. This is a long term independent contract role. The Case Manager is needed Monday to Friday.

Social Workers will need to be comfortable meeting consumers in their homes and in the community.

The Social Worker must be a Michigan resident, and possess a Michigan Social Worker License i.e., LLBSW/LBSW LLMSW/LMSW. QMHP certification (preferred)

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
* Provide remote community-based services and support to Individuals regarding self-advocacy, education, community awareness, medication management and support.
* Provide web-based primary and behavioral health integrated services
* Respond to the needs, desires, and interests of Individuals utilizing formal and informal assessment practices
* Provide weekly web-based interventions to individuals.

CLIENT'S REQUIRED EXPERIENCE & EDUCATION
* Minimum of a Bachelor's Degree in Social Work
* Michigan Social Worker License in good standing (LLBSW/LBSW LLMSW/LMSW)
* QMHP Certification (preferred)
