Blize Healthcare is looking for a Medical Social Worker to join our team. You will help patients and their families navigate the emotional, social, and financial challenges associated with a medical diagnosis or illness. Your role is to ensure patients' needs are met, promote health, and prevent disease.
Responsibilities:
- Evaluating the psychosocial functioning of patients and families.
- Providing individual or group counseling to address emotional, behavioral, and social issues.
- Coordinating care, connecting patients with resources, and advocating for their needs.
- Helping patients and families prepare for transitions from hospitals to home or other settings.
- Assisting patients in accessing benefits, support services, and other resources.
Required Skills and Qualifications:
- Master's degree in Social Work from an accredited program.
- CPR certification.
- Driver's License.
