Medical Social Worker (M.S.W.) – Per Diem

Position Qualifications

• Medical Social Worker with current licensure to practice professional social work in the state.

• Graduate of a Master’s Degree in Social Work (MSW) program accredited by the Council on Social Work Education (CSWE).

• Maintains a current CPR certification.

• Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.

• Medical Social Workers shall have a minimum of (a) one (1) year of experience as a social worker in a healthcare setting (hospital, clinic, or home health) within the last three (3) years OR (b) have completed a specialized medical social work internship. Minimum of two years’ experience in home health or hospice care is preferred.

• Self-directed and able to work with minimal supervision while managing a diverse caseload.

• Management experience not required. Responsible for collaborating with the interdisciplinary team and providing social work perspective to physicians, nurses, and therapists.

• Demonstrates excellent observation, psychosocial assessment, crisis intervention, and verbal and written communication skills.

• Shows ability to organize and prioritize workload independently and connect patients with community resources.

• Physical ability to perform home visits, which may include climbing stairs, navigating various home environments, and carrying equipment or supplies up to 30 pounds. Visual and auditory acuity to perform required psychosocial assessments and interventions.
