What You'll Do:
As a Home Health Occupational Therapist, you'll partner with the Case Manager to provide skilled services to individuals in their homes. You'll evaluate and address their needs, empowering them to live safely and productively. Your expertise will be invaluable in areas such as:
Home safety assessments and modifications.
Medication management strategies.
Wheelchair seating and positioning.
Pain management techniques.
Falls prevention programs.
Management of chronic diseases and dementia.
Family and caregiver training.
What You'll Bring:
Current, unrestricted CA Occupational Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources.
Understanding of individual development, human behavior, and the influence of culture on healthcare.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Home health care experience (highly preferred).
Experience with high-acuity patients (a plus).
Experience working with an interdisciplinary team (highly desired).
Case management fundamentals (highly desired).
Self-direction, empathy, and critical thinking skills.
Outstanding interpersonal and leadership skills.
Strong organizational skills.
