About the Role:
As an Occupational Therapist, you'll partner with Case Managers to provide skilled services to individuals in their homes. You'll evaluate their needs and develop personalized plans to help them improve their independence and quality of life. Your work may include addressing home safety, medication management, wheelchair positioning, pain management, falls prevention, chronic disease management, dementia care, and caregiver training.

What You'll Need to Succeed:
Current and unrestricted CA Occupational Therapist License.
Current BLS/CPR Certification.
Valid CA Driver's License.
Knowledge of local community resources.
Understanding of individual development and human behavior, including the impact of illness/injury and cultural influences on healthcare.
Knowledge of state and federal homecare regulatory guidelines (preferred).
Experience in home healthcare (highly preferred).
Experience with high-acuity patients (a plus).
Experience working with an interdisciplinary team (highly desired).
Case Management fundamentals (highly desired).
Self-direction, empathy, critical thinking skills, outstanding interpersonal skills, leadership skills, and organization.
Compensation:
The salary range for this position is $120,000 - $130,000 annually. Starting pay is based on relevant experience. We also offer generous bonus incentive plans, comprehensive health benefits, and 401(k) with up to a 4% match.
