About the Role:
As a Community Partner Care Coordinator, you will play a vital role in ensuring Medicaid, Medicare Advantage, and Dual Eligible Special Needs Plan (DSNP) members receive the care they need. Your main responsibility will be to create positive experiences for members by building trusting relationships and helping them access the right care at the right time. This includes connecting members to health plans, providers, pharmacies, and other community resources. You'll also address both medical and social needs (Social Determinants of Health), helping members navigate their healthcare journey with support and compassion.
Key Responsibilities:
Conduct outreach to educate and motivate members about available programs and services.
Assess members' needs and create personalized care plans to address their health goals. These assessments will be done via phone and/or text.
Collaborate with healthcare providers to coordinate care and schedule appointments as needed.
Help members become active participants in managing their health and overall well-being.
Identify and support members in addressing Social Determinants of Health (SDOH).
Maintain accurate documentation of all actions taken within the care management platform.
Regularly engage with members, families, providers, and other partners to ensure smooth coordination of care.
Work with a caseload of members, ensuring timely and effective communication.
Required Skills & Experience:
2+ years of experience in care coordination, case management, social work, pharmacy support, or a related field.
Fluency in English is required, and Spanish proficiency is a plus.
LPN or RN credential preferred.
Experience working with Medicaid or Dual Eligible patients is highly desired.
Ability to work remotely from a dedicated home office.
Located in Tennessee.
Additional Skills:
Strong interpersonal, communication (written and verbal), and problem-solving skills.
Comfortable working with customers via phone, text, and electronic communication.
Familiarity with Apple computers (e.g., MacBook).
Experience documenting in care management or electronic health record systems.
Skilled in motivational interviewing techniques.

Schedule & Work Environment:
Full-time, Monday through Friday, 8-hour shifts during regular business hours.
This position is remote, but you must reside in Tennessee.
