Salary : $60,000.00 to $76,000.00

Job Description:

The RN Patient Nurse Navigator Cardiology specializes in guiding patients and their families through the complexities of cardiology care. This role involves managing the continuity and spectrum of care, coordinating with multidisciplinary teams to support the patient's treatment plan from diagnosis through follow-up. The navigator acts as a consistent point of contact, aiding with education, resource management, and seamless transitions between care settings.

Key Responsibilities:

• Acts as a primary contact for patients and families, guiding them through the healthcare system.
• Coordinates and improves the efficiency of care, ensuring positive patient outcomes.
• Educates patients and families about treatment options and healthcare resources.
• Assists in the development and implementation of patient care programs.
• Analyzes patient data to improve care quality and close healthcare gaps.
• Collaborates with healthcare professionals to align resources with patient needs.
• Supports the development of educational and outreach programs.

Requirements:

• Must be a graduate from an accredited nursing program.
• Holds a current RN license from the South Dakota Board of Nursing.
• BLS Certification from the American Heart Association.
• Preferred Bachelor's degree in Nursing.
• 1+ years of Case Management Experience; 1+ years of Patient Care Experience
